Case Management (FamilyWorks)

FamilyWorks is a voluntary comprehensive case management program serving CHA’s entire population of public housing and 10/1/99 relocatee residents using temporary Housing Choice Vouchers.  Outcomes are measured according to distinct goals and focus on:

  • Employment preparation and retention
  • Education and job training
  • Youth opportunities
  • Permanent housing
  • Lease compliance
  • Clinical services
  • Senior support

 

Current FamilyWorks providers are:

  • Centers for New Horizons
  • Employment and Employer Services
  • Heartland Human Care Services
  • Metropolitan Family Services
  • Uhlich Children’s Advantage Network (UCAN)

 

Other CHA case management providers that use a model similar to FamilyWorks at mixed-income developments are:

  • Holsten Human Capital Development
  • Near West Side Community Development Corporation
