Program Coordinator, Senior Support Services - Resident Services

Minimum Salary: $34,455 - $35,000
Assists and supports the day-to-day activities and operations of the department in order to achieve target goals, objectives and outcomes. Accountable for monitoring, tracking, evaluating and reporting on the performance and progress of an initiative from inception to closeout. Assists in developing and implementing the policies and procedures to achieve the goals, objectives and outcomes of the initiative.
SUMMARY OF PRIMARY ACCOUNTABILITIES:
- Schedule and conduct home visits to determine needs, makes referrals, and respond to concerns.
- Coordinate programs, activities and events designed specifically for seniors.
- Develops print and distribute notices of events being held, collect monies, arrange transportation and assist in helping residents so that they may participate.
- Coordinate services for residents with Social Service Agencies.
- Assess all residents’ needs thru individual interviews and maintain a confidential case file on each resident.
- Develop linkages with the local community to expand services for residents.
- Act as liaison and advocate on behalf of the residents to public and private agencies.
- Assist the department staff in refining policies and procedures as necessary and as related to the program.
- Assist in the creation of the program evaluation process.
- Responsible for assisting management in identifying potential conflicts, problems or challenges of the work assignment and coordinates activities in order to resolve outstanding issues.
- Assists with coordination and preparation of department materials for distribution ensuring accuracy of the data.
- Assists Management with the coordination of administrative and operational activities of the initiative assigned.
- Responsible for attending meetings to address the department’s status on various events and social outings.
WORKING CONDITIONS
Some duties of this position are performed in an office environment under normal working conditions. The incumbent will also be required to travel to various sites, on and off CHA. The incumbent may be required to work extended hours, including weekends.
REQUIREMENTS
- College level coursework in the areas of Social Service, Business/Public Administration or related field, or the equivalent combination of education and experience
- Two to four years experience in coordinating community based initiatives
- Working knowledge of CHA policies and service contractual agreements
- Strong analytical, problem-solving, and organization skills
- Effective verbal, written, and customer relation skills
- Strong interpersonal skills
- Ability to exercise objective judgment, discretion and work on multiple initiatives simultaneously
- Advance skills with Microsoft Outlook, Word, Excel, PowerPoint, Access & data entry
- A valid state of Illinois Driver’s License
